Physical therapy is an effective treatment for a wide variety of physical dysfunctions. It is a crucial component of successful rehabilitation after an injury or significant medical procedure such as surgery, but it is also an excellent, non-invasive option for aches, pains and normal age-related physical degeneration. How to Make a Physical Therapy Referral

To start the process of referring a patient to us, please complete a referral form and either email it to pht.info@cascadehealth.org or fax it to (541) 636-3478.

If you are including a recommendation for physical therapy on hospital discharge orders, please refer to Cascade Health by name.

Your patient or their family members may also self-refer. If we receive an inquiry from someone who is your patient, we will contact you to coordinate care if the patient authorizes it.
