We work collaboratively with you and your physician to create a care plan that addresses all your recovery needs — from pain relief to restoring physical function, personal care assistance to speech therapy and wound care to help managing personal issues. Our goal is to not only help you regain health, strength and function, but to empower you and your loved ones so that you can continue to live your best life when our time with you is over.

Reach Your Health Goals with Home Health Care

We understand that medical rehabilitation can be stressful and work to provide comfort and peace of mind for both you and your loved ones. This begins with keeping you at home where you’re most comfortable and taking a whole-person approach to your needs. Cascade Health will assemble an experienced team of Home Health caregivers to assist you that may include a registered nurse, physical therapist, speech therapist, occupational therapist, medical social worker and home health aide. Based on your unique needs, they will:

✅ Assess, evaluate and treat your medical conditions including I.V. therapy, wound care, diabetes management, nutrition counseling and more.

✅ Establish a regiment of physical therapy exercises to help you regain strength.

✅ Retrain you in completing daily activities that may have been affected by your condition, including showing you how to use adaptive equipment if needed.

✅ Address speech, communication or swallowing changes and create a therapeutic plan to restore function.

✅ Help you and your family solve personal, financial or other problems and arrange for necessary medical supplies, equipment and community resources.

✅ Assist you with personal care, exercise, food preparation and minor household chores.

✅ Train you and your loved ones in how to use medical equipment, manage medications, make your home safer, prevent falls and more.

✅ Provide 24/7 support to you and your family.

How to Explore and Start Home Health Care

If you think you might benefit from Home Health care now, or have a planned procedure from which you’ll need to recover, please call us at (541) 228-3050 or email hh.info@cascadehealth.org. We can provide guidance about eligibility and help you with the referral process if appropriate. We are a nonprofit organization and our goal is to answer your questions and empower you in your health decisions. We will never pressure you to establish care with us.

What to Expect at Your Visits

Your first visit from Cascade Home Health nurse or therapist will likely happen within 24 hours of the time you’re discharged from the hospital or nursing facility. Your nurse or therapist will talk to you about your condition, symptoms, goals and living arrangements. They will ask and answer questions, speak with any family members involved in your care and conduct a review of your home’s safety. In combination with your doctor’s notes and recommendations, they will then identify treatments, tools and training that will help you manage your symptoms and reach your health goals.

They may begin initial treatments then or schedule additional visits based on your doctor’s recommendations. At each appointment, your provider will review your progress, address any concerns, update your care plan as needed and order any additional services they feel may help you reach your goals.
