Cascade Home Health gives you the freedom to stay in your home and maintain your independence while bringing highly personalized, expert care to your home. With 24/7 access to nursing care and training for family caregivers, you and your loved ones will rest easy knowing help is there if you need it. We provide home-based care to people in Eugene and Springfield, Oregon and the surrounding Lane County area, including: Cottage Grove, Junction City, Creswell, Monroe, Veneta, Noti, Blue River, McKenzie Highway, Harrisburg, Sweet Home, Oakridge, Coburg and Lowell.
Cascade Health’s experienced, friendly team of Home Health professionals includes nurses, therapists, medical social workers and health aides who will work with you to help achieve your health goals and feel your best.
What Is Home Health Care
Research shows that people typically recover faster and with fewer complications with Home Health than those who stay in the hospital — and most people strongly prefer staying in their own home to recovering in a nursing home.
Home Health is fully covered by Medicare, Medicaid/Oregon Health Plan and most private insurance plans.
Home Health is not the same thing as personal home care. Home Health is clinical care by licensed medical providers. Personal home care includes things like meal preparation, housecleaning and other non-medical services.
Home Health can be provided to you anywhere you call home.
Home Health does not replace other health care you are receiving — it provides only the services you need to regain your health and ability to function normally after illness, injury or surgery.
You do not have to be a certain age to receive Home Health. Anyone of any age who finds it difficult to leave home may be eligible to receive Home Health services.
You do not have to be bed-bound to be eligible for Home Health care services. If leaving home requires considerable effort, you may be eligible for Home Health.
Cascade Home Health is the only independent local nonprofit home health agency in Lane County and has been serving our community since 1982.
Signs that you should consider asking your (or your loved one’s) doctor for an exam to determine if Home Health is appropriate include:
✅ Recent diagnoses of a serious illness such as diabetes, heart disease, kidney disease, cancer, neurological conditions or dementia.
✅ Frequent visits to the hospital or doctor’s office.
✅ Pattern of missed appointments or obligations.
✅ Difficulty leaving home for care.
✅ Difficulty eating or swallowing.
✅ Difficulty managing medications or multiple conditions.
✅ Recent surgery.
✅ Decline in overall health or quality of life, including unintentional weight loss or gain, losing interest in activities, recent falls, bathroom accidents or poor hygiene.
✅ Some of these symptoms may also indicate the need for palliative care or hospice. Your doctor can help you decide which of these services is most appropriate for you.

Home Health care is provided on a short-term, intermittent basis based on need. If you require rehabilitative services after an injury, surgery or illness and are unable to easily leave home for follow-up care, you may be eligible for Cascade Health Home Health. A referral from your physician is required and you have the right to choose what company you would like to provide your care. We appreciate you asking for Cascade Health by name.
