Cascade Occupational Health is here to serve your workplace health needs. From pre-employment screening to physical therapy after a workplace injury, we are dedicated to helping you stay safe, healthy and able to work.
Below, you’ll find information and resources to help make your visit as efficient and pleasant as possible. If you have any questions or are not able to find the information you need, please call our offices before coming to the clinic. Some appointments require specific preparations or paperwork and if you have not followed required guidelines, we will have to reschedule your appointment. Thank you!
If you have an appointment, please arrive 15 minutes prior to your scheduled time to check in and complete any necessary paperwork.
If you cannot make your appointment, please cancel 48 hours in advance by calling (541) 228-3100 and choosing option 1.
You must bring:
✅ A valid form of identification
✅ Authorization for Services form (“Passport”) from your employer, if required (please check with your employer)
✅ Any visit-specific paperwork (please see Preparing for Your Appointment, below)
✅ Your health insurance card. This is required even if you expect workers’ compensation insurance to cover your visit.
When you arrive, you must check in. Our clinic is very busy, and we cannot monitor our lobby. If you do not check in, we will not know you are waiting, and you may miss your appointment. Please stand at the sign and wait to be called to one of the registration desks. We will check you in, review your paperwork and ask you to wait in our lobby until you are called back to an exam room.

Weapons-Free Zone
Weapons are prohibited on the premises of all Cascade Health facilities. No guns are allowed, regardless of whether an individual has a license to concealed carry. The only exception applies to uniformed police officers.
Mask Policy
At Cascade Health, the safety of our clients, patients, and employees is our top priority. Masks are no longer required in health care settings, but are encouraged in our clinics. If you are symptomatic, we may request that you wear a mask.
Proper preparation for your appointment will ensure that you are see as quickly as possible. In some cases, if you do not bring specific paperwork or materials with you, or do not come at a certain time, we will not be able to complete your exam and you will need to reschedule.

Preparing for Your Appointment
Employee Forms

Patients who require rehabilitative care such as skilled nursing or physical, occupational or speech therapy to recover following an illness, injury or surgery and are restricted in their ability to leave home are eligible for Home Health care.

Complete and bring the Medical Examination Report (MER) Form, MCSA-5875.
Register on the F.M.C.S.A. Commercial Driver’s License Drug & Alcohol Clearinghouse.
Drivers must only register.
Owner Operators must register as an employer and designate Cascade Health as your C/TPA (consortium/third party administrator).
If you have any of the following conditions, please bring the paperwork indicated. Failure to do so will prevent us from completing your physical.
Diabetes. Insulin-Treated Diabetes Mellitus Assessment Form, MCSA-5870
Sleep apnea. Contact your primary care provider or sleep specialist to get a readout from your CPAP machine and your last annual assessment.
High blood pressure. List of current medications.
History of cardiac events. If you have had a heart attack, heart surgery, stent placement, etc., you will need to bring records from your cardiologist regarding the status of your condition. If the event occurred recently, this may include a release to resume normal activities. Bring a copy of your most recent cardiac testing as well.
Recent major surgery (within 90 days). Bring a statement from your surgeon regarding your ability to drive safely.
Seizure. Bring your medical records regarding this condition. You will likely need to apply for an exemption or waiver.
Vision and hearing issues. Bring your corrective lenses, hearing aids and any applicable exemptions or waivers.
Current medication list. If you take anti-psychotics, opioids, tranquilizers or other sedating medications, bring a letter or your medical records from the prescribing physician that describes your diagnosis, stability of your medical treatment and a comment on the sedations.

Before making an appointment with Cascade Health:
Visit the F.A.A.’s MedXPress page and log in or create a new account.
Complete the questionnaire and note the reservation number you are provided.
Call us at (541) 228-3100 to schedule your appointment. Appointments are typically available 10 – 14 days from the time you call.
If you have any current medical conditions affecting your licensure, you must bring the related records from your physician with you to the appointment.

Complete and bring an Audiometric Data Sheet.
If you use a hearing aid, bring it with you.

Visual acuity: complete and bring an Audiometric Data Sheet and any hearing aids you may use.
Audio acuity: if you wear corrective lenses, please bring them with you.
Respirator fit: Shave the area under the seal of the respirator you use and bring the respirator with you to the exam.

If you wear corrective lenses, please bring them with you.

Complete an OSHA Respiratory Questionnaire Form. Email it to Cascade Health at [email], fax it to (541) 228-3107 or mail it to 2650 Suzanne Way, Suite 200.
A nurse will review your form and if you meet the criteria to use a respirator at work, a certificate will be mailed to your employer.
Schedule a fit test if required (check with your employer) by calling (541) 228-3100.

Shave the area under the seal of the respirator you use at work.
Bring the respirator you use at work with you to your appointment. After passing a fit test, you must use the exact same make, model, style and size respirator on the job.

Please be aware: 2-step TB tests cannot be done on Thursdays, or on Fridays when the following Monday is a holiday.

Vaccination record (if you do not have a record, we will check the Oregon Alert System). Depending on your history, we may recommend a “titer” test to see if you have immunity from previous vaccinations prior to giving you a new vaccination).
For the Covid-19 vaccine, if it is not your first dose, bring your vaccine card with you.
