Please review the Pfizer Fact Sheet and bring your insurance card, a signed consent form and your vaccine record card (if you will be coming for your second or third dose) to your appointment.
+ A two-dose primary series in individuals 12 through 15 years
+ A third primary series dose in individuals 12 years of age and older who have been determined to have certain kinds of immunocompromise, and:
+ A single booster dose in individuals 65 years of age and older
+ 18 through 64 years of age at high risk of severe COVID-19
+ 18 through 64 years of age whose frequent institutional or occupational exposure to SARS-CoV-2 puts them at high risk of serious complications of COVID-19 including severe COVID-19

Cascade Health COVID-19 Safety Protocols
At Cascade Health, the safety of our clients, patients, and employees is our top priority. We continue to implement full COVID-19 safety protocols at all of our facilities.
We are open in all areas of service, however please note that some services are being provided via telehealth applications. Please contact our departments directly for more information. It is important to note that we are NOT providing testing for COVID-19. If you are ill or feeling unwell, please contact your primary care provider or Lane County Public Health at 541.682.1380 for testing options.
If you have severe symptoms or are experiencing a medical emergency, please call 911.
If you are feeling well but have general questions, please visit the Oregon Health Authority Website at Oregon Health or Lane County Public Health at Lane County Health. These resources offer helpful information on avoiding COVID-19, symptoms to watch for, what to do if you think you might have the virus and more.
