Travis Brooke

Travis Brooke

Director of Occupational Health

  • Travis leads Cascade Occupational Health, overseeing numerous programs including occupational medicine, physical therapy, employee testing, mobile health, wellness and onsite clinics. Travis specializes in identifying employers’ immediate needs and developing cost-saving solutions that benefit worker health while decreasing workers’ compensation costs. A graduate of the University of Oregon with a bachelor’s degree in sociology, Travis is a Eugene native who enjoys spending time with his family, golfing, skiing, volunteering and supporting the Oregon Ducks.

Manager of Mobile Health

  • In 2022, Gabrielle joined Cascade Health, bringing her experience as an emergency medical technician and health care management with her. She helps support the Cascade Mobile Health program. She received her bachelor’s in psychology and minored in philosophy at Cal Poly Pomona. When Gabrielle is not working, she can be found reading, baking, collecting houseplants and raising three children. She is an enthusiastic lifelong learner and enjoys being part of a team working to support clients and patients in her community.

  • Travis joined Cascade Health in 1999 as part of the occupational health group. Prior to that, he completed a Bachelor of Science in occupational from the University of Puget Sound. He has worked in a variety of clinical positions across the United States and became most interested in orthopedic evaluation and treatment. He enjoys utilizing the application of orthopedic evaluation and manual treatment to help workers restore their strength, mobility and function. He and his wife have two daughters and the family loves camping, hiking, and cruising through the dunes and mountains on side-by-sides. Travis also enjoys hunting and fishing.

Manager of Occupational Health

  • Michelle joined Cascade Health in 2021 and manages the Occupational Health department. Michelle earned her bachelor’s degree at the University of Oregon in exercise and sports science. She loves running, cross-fit and hiking in her free time as well as spending time with her family and traveling abroad. Michelle was a founding member of the Cascade Medical Team, a group of physicians and medical professionals who do medical mission trips to provide care in Guatemala and is a Paul Harris Fellow with Rotary International. She is eager to partner with our community to keep people happy, healthy and working.

Benjamin Gerhart, N.C.M.A., L.X.M.O.

Assistant Manager of Occupational Health

  • Ben joined Cascade Health in 2017. With over 15 years working with computers, he went back to school in 2015 to change his profession, studying to become a Medical Assistant and X-Ray Technician. With his computer background he looks for ways to help Cascade Health strive in the use of technology to improve patient and company interactions. He enjoys going on-site to various companies assisting in Hearing, Spirometry and Mask Fittings tests. On his off time, he enjoys music, having played the Viola since High School and playing in a Japanese Taiko group since 2012. He enjoys bike riding, kayaking, and camping with his wife and family.

Richard Abraham

Richard Abraham, M.D.

Physician

  • Dr. Abraham has been a practicing emergency and occupational medicine physician with Cascade Medical Associates since 1979. He graduated from the University of Western Ontario Schulich School of Medicine & Dentistry, is board certified in occupational and environment medicine and is a fellow of the American College of Emergency Physicians. He is the founding partner of Best Med urgent care and Monarch Medical Weight Loss. He is also a founding director and board member of Summit Bank and is a board member and prior chair of Cascade Health – serving on the boards of McKenzie Willamette Medical Center and Attwill Medical Solutions.

Jonell Blase, D.N.P., F.N.P.-B.C.

Physician

  • Jonell Blase completed her undergraduate degree in nursing science with the University of Alaska, and master’s and doctorate nursing degrees with Oregon Health and Sciences University. Jonell has provided healthcare in the Willamette Valley since 1991, working in home health, family medicine, internal medicine, emergency medicine, urgent care, and occupational medicine. She joined Cascade Medical Associates in 2019 where she cares for injured workers and assists their return to work. Jonell is a certified commercial driver medical examiner. When not in clinic she enjoys her family and friends and tending her small farm with her giant, furry dog at her side.

Mitch Boriskin, F.N.P.

  • Mitch Boriskin has served Lane County as a family nurse practitioner since 1985. He spent more than 20 years caring for patients in rural Oakridge and more than 10 years in the emergency room at Cascade Medical. Mitch was a founding partner of Eugene Urgent Care (now BestMed). He is married with three children and one grandson.

Lee Davidson

Lee Davidson, M.D.

Physician

  • Lee Warren Davidson, M.D. joined Cascade Medical Associates in 1999 as an emergency physician at McKenzie-Willamette. There, he gained experience in occupational medicine and transitioned to practice exclusively at Cascade Health in 2017. He enjoys helping injured workers recover and can relate to the demands of their job. He has been involved with local EMS and is the past chair of the board at McKenzie-Willamette Medical Center. When not at work he enjoys time with friends and family, most notably watching his son play high school soccer.

Natalia Hayes

Nurse Practitioner

  • When Natalia Hayes was a child, she would use stethoscopes and bandages borrowed from her mother, a nurse, to treat her dolls. It was therefore no surprise when she completed the registered nurse program at Lane Community College in 2007 and went on to receive her Bachelor of Science in Nursing from Linfield College in 2009. Since then, Natalia has received her Nurse Practitioner degree from Maryville University. Natalia joined Cascade Health in 2018 and enjoys being part of a supportive team helping people heal. In her free time, Natalia is a piano player and a singer, and loves spending time with her family.

Luci Kovacevic

Luci Kovacevic, M.D., M.P.H., D.A.B.P.M.

    Board certified in occupational medicine, Dr. Kovacevic is Cascade Occupational Health’s medical director. He joined Cascade Medical Associates in 2007 to provide occupational health services and his training includes commercial driver exams, FAA aeromedical exams, medical review of drug tests, NFPA 1582 physicals and OSHA-mandated exams. He previously served as a U.S. Navy general medical officer and developed an interest in the operational needs of special communities while stationed with Marine Aviation Logistics Squadron-16. Dr. Kovacevic and his family enjoy the outdoors as well as the arts and entertainment that Eugene has to offer.

Anne Christensen

Anne Christensen, M.D.

  • Dr. Christensen is board certified in occupational and environmental medicine and has practiced occupational medicine since 2006. Dr. Christensen grew up in Lane County, attended medical school at University Alabama Birmingham, and completed her residency in occupational medicine and her master’s degree in public health at Harvard. She has more than a decade of expertise in health policy and serves on the board of Public Health International and is a member of the International Corporate Health Leadership Council. Partnering to improve worker wellbeing in communities and workplaces is her favorite part of occupational medicine.

Colleen Tuman

Colleen Tuman, N.P.

  • Colleen Tuman joined Cascade Health as nurse practitioner in occupational medicine in 2016. She graduated with a Bachelor of Science in Nursing from the University of Virginia in 2005 and began her career as an intensive care registered nurse at Georgetown University Hospital directly afterward. She received her master’s in nursing from Virginia Commonwealth University in 2012, specializing in family medicine. Colleen finds working in Occupational medicine extremely rewarding because of the motivation of the patients to return to their optimum health and lifestyles.

Lisa Kovacevic

Lisa Kovacevic, R.N., N.P.

    Lisa Kovacevic joined Cascade Health in 2008. She graduated from the University of Virginia Nursing School with her Bachelor of Science in Nursing and received her master’s degree from the University of San Diego. She began working in occupational medicine after serving as an officer in the US Navy Nurse Corps. She is a certified D.O.T. medical examiner and a member of the American College of Occupational and Environmental Medicine for nurse practitioners. Having grown up on the East Coast, Lisa enjoys exploring the Pacific Northwest and particularly enjoys hiking and stand-up paddle boarding.

    Dr. Pederson joined Cascade Health in 2022 after retiring from his position as the medical director for Samaritan Occupational Medicine. He holds a bachelor’s degree at the University of Oregonand a Doctor of Medicine from New York Medical College. He also earned a master’s degree in public health from Uniformed Services University of the Health Sciences. Dr. Pederson has extensive training in occupational medicine and is certified as a medical examiner for both the D.O.T. and F.A.A. In his free time, Dr. Pederson enjoys the outdoors, woodworking and spending time with his family.

Adam Clark

  • Scott joined Cascade Health in 2001 and again in 2016. He has a bachelors degree from the University of Oregon and a Masters in PT from Nova Southeastern University. Scott has worked in a variety of clinical settings from Acute care, home health to his current outpatient position. His approach to physical therapy is by applying science-based interventions combined with encompassing mind, body and spirit in a holistic approach. He and his wife have two sons and have lived in Eugene for more than 20 years. Scott and his family are avid baseball fans and spend much of their free time watching their boys play baseball. Scott’s family also volunteers at local schools and enjoys fishing and hunting.

Physical Therapy Assistant

  • Adam began working at Cascade Health in. He received his training in the Air Force and became involved in rehabilitation in that setting and has also worked as a physical therapist assistant and a chiropractic assistant. He finds helping people return to function so they can get back to work quickly very satisfying. In addition to his clinical work at Cascade Health, he has served on the safety committee since 2006, including as president for three years. In his free time, he enjoys spending time with his family and riding motorcycles.

Leslie Hoffman, O.T

Hand Therapist

  • Leslie joined Cascade Health in 2021 because she was drawn to work for a local non-profit organization with a strong focus on patient care and community involvement. She loves getting to know her patients and working collaboratively to figure out how to improve their injuries. She holds a Bachelor of Science in occupational therapy from Colorado State University and is a certified hand therapist. She loves off-roading, hiking, watersports, skiing, camping, reading, cooking, rock hounding and wood working. She and her husband have two daughters and numerous beloved pets.

Lena Houston-Davisson

Physical Therapist

  • Lena is a licensed physical therapist and joined Cascade Health in 2000. She enjoys assisting employers and workers find ergonomic and injury prevention solutions in a wide variety of settings. Lena earned a Bachelor of Science in psychology from the University of Oregon and a Master of Physical Therapy from the University of Puget Sound. In her free time, she enjoys gardening, walking in nearby nature and the antics of her two cats. She completed CERT (Community Emergency Response Team) training in 2017, is a licensed amateur radio operator – providing emergency communication among local medical facilities and throughout the state, as needed.

  • Travis has been working with Cascade Health since 2005. He earned his Doctor of Chiropractic from University of Western States (formerly Western States Chiropractic College) in 2003, and then worked in private practice before joining Cascade Health. Travis likes working with patients and the satisfaction of seeing them get back to doing the things they love. In his free time, he enjoys ice hockey, skiing, and mountain biking.

Tyler Tylinski

  • Tyler has been working with Cascade Health since 2017. After earning his doctorate in physical therapy from Carroll University in 2015, he began his career in the Chicago Land area. He is currently training with the McKenzie Institute to become credentialed in Mechanical Diagnosis and Therapy. Tyler likes working with patients and the satisfaction of seeing them get back to doing the things they love. In his free time, he enjoys salsa dancing, skiing and mountain biking.

Christine Veloon

Occupational Therapist

  • Christine has been a licensed occupational therapist for patients and families in health care settings since 1982. She has worked at Cascade Health in outpatient therapy and home health care since 2005. Christine enjoys assisting people coping with illness and injury to help them regain function and lead productive lives. She and her partner are enjoying semi-retirement and love to travel, garden and spend time with their Sheltie, Rosie. Christine also loves pickleball, quilting and walking with friends.
