Cascade Home Health can help you deliver excellent rehabilitation outcomes for your patients following an illness, injury or surgery. Home Health care has been shown to be as effective as that provided in skilled nursing facilities, while enabling people to stay in their own homes as most prefer.

When to Refer Patients to Home Health
Home Health care is intended to help patients who need intermittent skilled nursing care, physical therapy, speech/language therapy or occupational therapy and are unable to easily leave home to access such services. Home Health care is often needed following an acute illness, new diagnosis, injury or significant medical procedure, such as surgery.
Medicare-specific criteria for eligibility include:
The patient must be homebound.
The patient must require skilled services.
The patient must need only part-time care.
The care the patient needs must be reasonable and medically necessary.
The patient must be under the care of a physician.
The care must not be duplicative.
The patient must live in an environment that is safe for both the patient and Cascade Health staff.
How to Make a Home Health Care Referral
Cascade Health’s Home Health team works quickly to process your referral and begin treating your patient. You may start the process by completing the Cascade Home Health Referral Form and faxing it to (541) 228-3182 or by calling us at (541) 228-3050. If you call, please be prepared to provide the following information:
Patient name, address and phone number.
Patient date of birth.
Diagnosis.
Referring contact and phone number.
Attending physician’s name.
Primary contact/caregiver name, phone number and relationship to the patient.
Medicare/insurance ID number, phone number, policy number and name of policyholder.
If you are including a recommendation for Home Health on hospital discharge orders, please refer to Cascade Health by name.
Your patient or their family members may also self-refer. If we receive an inquiry from someone who is your patient, we will contact you to coordinate care.

Patients who require rehabilitative care such as skilled nursing or physical, occupational or speech therapy to recover following an illness, injury or surgery and are restricted in their ability to leave home are eligible for Home Health care.

No. Home Health care is intended to provide short-term intermittent assistance to aid in the rehabilitation process. Patients with serious illnesses who have longer-term needs may be eligible for referral to Palliative Care. Patients who have a prognosis of six months or less and need help may be eligible for Hospice. Anyone can make an initial referral to Home Health - a primary physician or specialist, such as an oncologist, the patient, a family member or a friend. However, to qualify for Home Health service, a physician must certify that the patient is home-bound. A patient does not need to be bedridden to qualify, but leaving home must require “a considerable and taxing effort.”

Cascade Health does not offer home care help, though our Home Health aides can provide minimal assistance with personal care and household chores if their primary duties (bathing, bathroom assistance) allow time. We can, however, help guide your patient to local caregiving resources and our social worker helps to seek out funding sources for patients as needed and available.
