Staff RN Triage

Responsible to: Hospice Clinical Manager
Hours: 40 hours/week
Schedule: Monday - Friday 8 hours/day
Pay Range: $41.19 - 63.50

Job Summary:

The registered nurse functions as a member of the hospice team, providing skilled care to hospice patients in their place of residence. This is a float position that supports the clinical team from the office by triaging calls and may conduct urgent patient visits.  All care is given in accordance with physician’s orders.  The nurse evaluates patients, plans and implements effective therapeutic interventions based on physical assessment, sound judgment, and psychological sensitivity.  Understands the hospice philosophy and provides treatment to dying patients.  Utilizes current standards of practice and follows departmental policies and procedures.  Complies with applicable accreditation and regulatory requirements governing the practice of nursing.  Maintains a high level of professional expertise and assumes responsibility for professional growth and development.  When appropriate, the RN provides supervision to the hospice aide assigned to the patient.  Participates in the orientation of new employees.oyees.

Education & License Requirements:

Current licensure in the State of Oregon as a Registered Nurse and a current CPR card.

Minimum Qualifications & Experience:

Two years of Hospice case management or triage experience.  Two years recent (within the last five years) med/surg experience. Needs to be comfortable with electronic documentation and able to document using a laptop computer.  Must have a current driver’s license, insurable driving record, a licensed vehicle for transportation, and a means of communication.  Desired skills include: High degree of written and verbal communication, flexible, team player, with strong problem-solving skills.

Essential Job Functions:

  • Perform thorough assessment of patients, including pain and symptom management needs, primarily by phone.

  • Triage calls to determine level of urgency and priority for intervention or response.

  • Collaborate care coordination with interdisciplinary team.

  • Provide guidance on symptom management and make recommendations for medication adjustments or other interventions as necessary.

  • Respond to urgent situations by coordinating with case managers and leadership or providing urgent visits if needed.

  • Provide and document patient and family education associated with a terminal illness, accurately and timely.

  • Work independently with effective time management.

  • Demonstrate sound judgment.

  • Maintain clear and compassionate communication with patients and care team.

  • Participate in conferences, staff meetings, and in-services as assigned.

  • Organize, prioritize, and delegate patient care needs as they arise. 

  • Move and transfer patients.

  • Administer treatments and use health care related equipment.

  • Develop a good working relationship with referral sources.

  • Drive an automobile safely.

  • Share on-call responsibilities.

  • Perform other duties as assigned.

Marginal Job Functions:

Answer phones.  Clerical duties.  Move furniture, equipment, and supplies.  Post and access schedule.
