On-Call Staff LPN

Responsible to: Hospice Clinical Manager              
Hours: Variable
Schedule: Rotating On-Call Coverage, 4:30pm-8:30am
Pay Range: $26.91 - 41.51

Job Summary:

The on-call licensed practical nurse functions as a member of the Home Care team, providing skilled care to Hospice & Home Health patients in their place of residence during the evening and night hours based on patient needs.  All care is given in accordance with physician’s orders.  The nurse evaluates patients, plans and implements effective therapeutic interventions based on physical assessment and sound judgment following the care plan. Understands the Hospice philosophy and provides treatment to dying patients.  Utilizes current standards of practice and follows departmental policies and procedures.  Complies with applicable accreditation and regulatory requirements governing the practice of nursing.  Maintains a high level of professional expertise and assumes responsibility for professional growth and development.  May be assigned patient care visits for existing patients.  The on-call nurse will provide visit coverage for department non-business hours from 1630 to 0830 and will receive a lump sum payment of $100 for each on-call shift. If the nurse is called to make one or more home visits during an on-call shift, they will be paid a minimum of three (3) hours at one-and-one-half times their straight-time rate of pay.

Education & License Requirements:

Current licensure in the State of Oregon as a Licensed Practical Nurse and a current CPR card.

Minimum Qualifications & Experience:

Three years recent (within the last five years) med/surg experience, or one year (within the last three years) critical care experience. Two years Hospice or Home Health experience preferred.  Needs to be comfortable with electronic documentation and able to document in the home using a laptop computer.  Must have a current driver’s license and insurable driving record, a licensed vehicle for transportation, and a means of communication.  Desired skills include: High degree of written and verbal communication, flexible, team player, with good case management.

  • Ability to drive an automobile safely.

Marginal Job Functions:

Answer phones, clerical duties – put together charts, move furniture, equipment, and supplies.  Post and access schedule.  Light mechanics, instrument maintenance.
