Job Summary: Responsible for the 24-hour operation of the Home Health department, including planning, organizing, directing, coordinating, and evaluating direct and indirect patient care. Assume responsibility for day-to-day operational affairs of the department. Understand and comply with applicable accreditation and regulatory requirements governing home health. Develop and manage departmental budget. Track, manage and oversee clinical activities recorded in electronic medical record. Maintain a high level of professional expertise and assume responsibility for personal and departmental professional growth of this diverse department. Provide leadership for skilled nursing, rehabilitation, home health aides, and social work services. May require delivery of skilled nursing care when staffing requires supplementation. Position is assisted in managing the department by a Clinical Assistant Manager and Compliance Coordinator. Will participate in the hospice and home health management on call responsibilities on a rotational basis. This position requires proof of full COVID-19 vaccination at time of offer.

Education & License Requirements: Bachelor’s degree required and current Oregon licensure as a Registered Nurse.

Minimum Qualifications & Experience: Minimum of five years clinical nursing experience in home health, with two years of leadership/management experience required. Must have current driver’s license and availability of insured vehicle for transportation. Desired skills include: Clinical excellence, leadership abilities, teaching competency, critical thinking, and excellent communication skills. In-depth knowledge of current home health care concepts, trends and theories, reimbursement issues, and methods for delivery of home health services. Supportive of the philosophies of Cascade Health and Home Health Services. Ability to work as a member of a team.

Essential Job Functions:

  • Maintain knowledge base of current home health practices.
  • Develop and update schedules of clinical staff.
  • Assist with conflict resolution and peer communication.
  • Ensure monetary compliance with regulations and contracts.
  • Work independently, using good time management and sound judgment.
  • Use excellent communication skills: verbal, written, electronic.
  • Write and update policies and procedures.
  • Take vital signs and use medical equipment.
  • Participate in conferences, meetings, and in-services.
  • Organize, prioritize, and delegate routinely and in crisis.
  • Demonstrate clinical nursing skills, including CPR, response to emergencies, and patient assessment.
  • Exhibit good teaching skills for staff, patients, and families.
  • Use computer frequently.
  • Drive an automobile safely.
  • Hire, train, counsel, evaluate, and terminate staff.
  • Assure compliance with regulations and contracts.
  • Assist with organizing orientation process for new staff.

Marginal Job Functions: Clerical duties. Assist in moving office furniture, light repairs. Answer phones.
