AT HOME & ON CALL SERVICES

As always, we coordinate care 24 / 7 / 365 and will be here to serve you.

This includes Home Health, Hospice, the Pete Moore Hospice House, On Call Counseling, and Mobile Health.

ON SITE HEALTH SERVICES

• December 24th: Limited Counseling Appointments – Other departments are Closed
• December 25th: Closed
• January 1st: Closed

This applies to Workplace Health Clinic, Behavioral Health & EAP, Diabetes & Nutrition Education, Workers Action Program & Physical Therapy.

4J WELLNESS CLINIC

Clinic will be closed from December 24th – January 1st.

SPRINGFIELD WELLNESS CLINIC

• December 24th – 26th: Closed
• December 27th – 28th: Open for regular services
• December 31st: Clinic will be open until noon
• January 1st: Closed
