Cascade Home Health wins national award
Cascade Health Solutions’ Home Health agency was recently awarded 2011 Home Care Elite status for being in the top 20th percentile of all agencies in the nation that are achieving excellent quality outcomes. This designation was awarded to only eight agencies in Oregon; Cascade Home Health was the only agency awarded in Lane County.
This award demonstrates our ability to ensure that patients are cared for in the comfort of home as they heal. In addition, it shows that we are constantly striving to prevent re-hospitalizations, which has been identified by Congress and President Obama as a source for reducing Medicare spending.
Although readmitting a patient to a hospital may be appropriate in some cases, many agree that reduced re-admissions to hospitals will help contain Medicare costs and improve patient care.
We work to reduce hospital re-admissions in a variety of ways by:
• Communicating with the patient’s doctor
• Conducting safety evaluations
• Working closely with family members and caregivers
• Following a plan of care and re-evaluating the plan, as needed
• Providing high-quality home health services, delivered by a team that prides itself on its ability to listen, communicate and deliver compassionate, skilled care
This is one of the catalysts that have led hospitals and physicians to reach out to quality home health agencies for help in monitoring their patients’ health following hospital discharge.
The Medicare Payment Advisory Commission (MedPAC) reported that in 2005, 17.6 percent of hospital admissions resulted in re-admissions within 30 days of discharge, 11.3percent within 15 days, and 6.2 percent within 7 days.
Cascade Health Solutions’ Home Health Agency is among the top 5th percentile in the nation for having the lowest re-hospitalization rates.
In addition, we met or exceeded the Oregon state average for every quality measure publicly reported.
