Provider (Nurse Practitioner or Physician Assistant)

Hours: 32 hours/week

Pay Range: $49.27 - 73.91

Job Summary:

Responsible for the continuous and comprehensive management of a broad range of health care issues including promotion and maintenance of health, prevention of illness and disability, management of health care during acute or chronic phases of illness, and coordination of care for the employees and dependents of the City of Springfield and Springfield School District.  The Provider will work independently and will coordinate care with available resources to ensure that appropriate care is provided.  Will act as a resource to employees regarding wellness and lifestyle changes to assure optimum health. Our clinic is low-volume, with high patient engagement. New patient visits are scheduled for a full hour and routine visits are scheduled for 30 minutes. A typical provider will see 10-14 patients a day, providing excellent engagement and patient experience. No call coverage and paid admin time. Education & License Requirements:

Must be a graduate of an accredited school of professional nursing or physician assistant program.  This position requires current State of Oregon NP or PA licensure, prescriptive drug privileges, a DEA number, and current CPR card. 

Minimum Qualifications & Experience:

Two years of clinic experience preferred.  Desired skills include:  Verbal and written communication, problem solving, decision making, planning and organizing expertise.  Ability to assess and effectively resolve difficult clinical situations.  Must be able to maintain a high standard of professionalism in all activities undertaken within the Springfield Wellness Center.  Ability to work independently, as well as within a team framework.  Must manage challenges independently and confidently provide services to individuals in this healthcare setting.

Essential Job Functions:

  • Provide primary and urgent medical care services for patients utilizing the Springfield Wellness Center.

  • Apply a variety of clinical skills, including health status assessment, screening for disease, provision of appropriate treatment, education, and healthcare guidance to patients.

  • Order pertinent screening and diagnostic tests, assess findings, and implement proper plan of care. 

  • Arrange appropriate and timely referrals to outside medical resources, as indicated for complex situations.

  • Ensure complete and timely patient care documentation.

  • Function within the scope of practice for Nurse Practitioners or Physician Assistants as outlined by the Oregon State Board of Nursing or Oregon Medical Board.

  • Maintain a high level of professional expertise and assume responsibility for professional growth and development.

  • Provide quality clinical services in a caring, respectful manner.

  • Stand, sit, walk, or utilize other means of mobility in a clinic setting.  Assist patients with mobility in preparation for examinations or treatment needs.

  • Additional duties and responsibilities as assigned.
