Medical Social Worker

Responsible to: Hospice Manager              
Hours: Resource
Schedule: 8 hours/day
Pay Range: $30.01 - 45.01

Job Summary:

Function as an active member of the Hospice interdisciplinary team and render services in accordance with the plan of care in the patient’s place of residence.  All care is given in accordance with physician’s orders. 

Education & License Requirements:

Master’s degree from a school of social work accredited by the Council on Social Work Education is required and a valid CPR certification.

Minimum Qualifications & Experience:

Must have a current driver’s license, insurable driving record, insured vehicle for transportation, and a means of communication.  Experience with terminally ill patients and their families is highly desirable. 

Essential Job Functions:

  • Evaluate patients, plan and implement effective therapeutic interventions based on assessment, sound judgment, and psychological sensitivity. 

  • Understand the hospice philosophy and provide care to dying patients, their families or caregivers. 

  • Utilize current standards of practice and follow departmental policies and procedures. 

  • Comply with applicable accreditation and regulatory requirements governing the practice of social work and the National Social Work Code of Ethics. 

  • Maintain a high level of professional expertise and assume responsibility for professional growth and development. 

  • Provide supervision to students assigned to the department, when appropriate.

  • Maintain strict confidentiality.

  • Organize, prioritize, and delegate at a fast pace.

  • Communicate clearly, including verbal, written, telephonic, and electronic.

  • Maintain knowledge base of current community resources.

  • Work within organizational policies and contracts.

  • Coordinate home visits with other staff schedules.

  • Educate and train Hospice team and community members.

  • Work independently, remain flexible, and maintain good time management.

  • Negotiate terrain and physical obstacles to reach patients and safely drive an automobile.

  • Participate in conflict resolution, problem solving, peer communication, crisis management, and counseling.

  • Attend agency conferences, department meetings, and in-service training.

  • Serve as liaison between patient’s caregivers and physicians.

  • Develop good working relationships with referral sources.

  • Utilize computer systems to complete documentation.

  • Demonstrate sound judgment.

  • Assist patients and caregivers with interface of community and government organizations

Marginal Job Functions:

Participate in the orientation of new employees. Clerical duties related to patient care, including chart creation.  Assist with moving furniture, equipment, and supplies. Post and access schedule.
